In accordance with New York and Pennsylvania state statutes, notice is hereby given that Dr. Matthew Eernisse of Community Eyecare Specialists, has left the practices located at Seneca (2A Park Way, Seneca, PA 16346), Olean (2605 W State Street, Olean, NY 14760), Jamestown (462 Fairmount Ave. Jamestown, NY 14701). All patients may retrieve their medical records from Community Eyecare Specialists at any of the locations listed above or by calling (814) 677-6404. Dr. Eernisse’s last day at the practice was May 7, 2025. It is important for existing patients to schedule an appointment within the next 30 days either with one of Community Eyecare Specialists’ physicians or an outside eyecare doctor to ensure they are receiving ongoing care.
