One does it all: HomeCare & Hospice launches centralized hospice referral number

ALLEGANY — HomeCare & Hospice announces the launch of a simplified, streamlined way to connect families and providers with compassionate hospice care across Genesee, Wyoming, Cattaraugus and Allegany counties.

Effective immediately, all hospice referrals can now be made by calling (716) 372-5735 and selecting option #2. One call. One team. One mission.

“For over 50 years, HomeCare & Hospice has walked alongside families with expert care and heartfelt compassion,” said Melissa Sullivan, CEO of HomeCare & Hospice. “This change eliminates confusion about which office to call, ensuring families get timely support, no matter where they are.”

HomeCare & Hospice has served the region since 1971, offering comprehensive hospice and palliative care services that focus on comfort, dignity and personalized support. The new centralized number — backed by a dedicated team in the Allegany office — will help connect families more quickly with the resources they need.

“Our administrative assistants in each office will continue to support callers,” explained Nikki Wiseman, Chief Nursing Officer. “But this central point of contact will help us respond faster and more efficiently, getting care to patients and families sooner.”

The new central hospice referral contact info is (716) 372-5735 ext. 2 for phone, (716) 899-1872 for fax, referrals@homecare-hospice.org for email and homecare-hospice.org online. If you’re unsure which county the offices are located in, one number does it all.

This centralized system makes it easier for hospitals, skilled nursing facilities, clinics and private providers to connect patients with expert end-of-life care. HomeCare & Hospice officials said the clinical team is ready to respond with compassion, clarity and prompt attention.

For more information, please call (716) 372-2106 or email snegron@homecare-hospice.org.
