Community Day: Interfaith Caregivers welcomes Medicaid Social Care Navigator
OLEAN — Interfaith Caregivers is excited to welcome Angie Ritchie to its team as a full-time Medicaid 1115 Waiver Social Care Navigator. Angie brings a wealth of passion and dedication to helping individuals and families in our community connect to critical services that support better health and well-being.
In her new role, Angie will serve as a Social Care Navigator, she will work closely with Medicaid members, providing guidance and access to essential supports including food and nutrition, housing, transportation services.
Angie’s primary mission is to help Medicaid-eligible individuals navigate the often-complex web of social needs. She can screen and connect clients to supporting agencies to obtain nutritious meal deliveries, personalized diet and nutrition planning, cooking supplies, grocery shopping education, non-medical transportation, access to affordable housing, and home modifications such as grab bars, ramps, or air filtration systems.
Having a stable home, nutritious food, and support with daily essentials can have a profound impact on health. Angie’s work ensures that our community Medicaid members are not alone in addressing these challenges.
